Insurance
As a licensed
professional counselor, my professional services
qualify for client reimbursement, and may be covered
in full or in part by your health insurance or
employee benefit plan.
As an “out-of-network provider," I collect
fees directly from you, however many plans allow you
to apply for reimbursement using the billing
invoices I will provide for you. I
encourage you to contact your insurance company
prior to your first appointment with me to determine
your outpatient mental health benefits. When you
contact your insurance company, here is a list of
questions that may be helpful for you to ask:

Do I have mental
health benefits? What is my
deductible and has it been met?

How much do you pay
for out-of-network counseling/therapy service?

Is there a limitation
on how much you will pay per 90-minute session
(service code 90791 and 90847)?

Is approval/referral
required from my primary care physician?

How many
psychotherapy sessions per calendar year does my
plan cover?

When does my
plan’s calendar year begin?

How can I get a
copy of my claims form?
Make sure you get
the Agent’s name and direct phone number for any
follow-up questions you may have because oftentimes
different agents have different answers for the same
questions.
Payment
Session fee is due prior to the start of each
session. Electronic payments through Zelle.pay
is the preferred method. Venmo.com is an
alternative if your bank does not participate with
Zelle.pay. I do not accept credit cards to
keep session costs down.
Cancellation/Postpone Policy
If you do not show up for your scheduled appointment
and/or you have not notified me your need to
postpone/cancel at least 48 hours in advance, you
will be required to pay the full cost of the
treatment as booked since that time was reserved for
you and was not available to other clients. Emergency
and/or uncontrollable circumstances are waived.
A make-up session is expected
within the same month for any cancelled session. This is to
prevent therapeutic atrophy.
Final Session
Your final session is conducted in
person, not via email or phone call. This is to
provide you with closure and any outstanding work.
